Harwich, MA (April 5, 2025) – A fatal head-on crash in Harwich late Thursday afternoon has resulted in the death of a Hyannis man and the arrest of another Cape Cod man on multiple serious charges, including manslaughter and operating under the influence.
According to Cape & Islands District Attorney Robert J. Galibois, the crash occurred around 5:26 p.m. on Route 6. Upon arrival, Massachusetts State Police found two vehicles — one positioned in the median between the eastbound and westbound lanes, and the other in the eastbound breakdown lane, facing west against a guardrail.
Mario Da Paixao Andrade, 48, of Hyannis, was pronounced dead at the scene. The other driver, Zachary Fena, 49, of Centerville, was transported to Cape Cod Hospital for treatment.
Fena was later arrested and charged with manslaughter while operating under the influence, motor vehicle homicide while OUI, and several other related offenses.
