Toluca Lake, CA (September 19, 2025) – The Los Angeles County Medical Examiner’s Office has confirmed the identity of the man killed in a rollover crash on the 134 Freeway last month as 51-year-old David Smith.
According to the California Highway Patrol, the fatal crash occurred around 12:11 a.m. on August 15 on the westbound Ventura (134) Freeway near Cahuenga Boulevard. Two SUVs collided under unclear circumstances, causing one of the vehicles to overturn onto its roof.
Smith was pronounced dead at the scene. Another individual sustained injuries and was transported to the hospital by Los Angeles Fire Department personnel. The extent of their injuries remains undisclosed.
The Hollywood Way and Buena Vista on-ramps to the westbound 134 Freeway were shut down for several hours as Burbank Police and Caltrans managed the accident scene.
